We send nurses to your home to provide health care.

To be eligible for these services, you must be referred by a doctor, podiatrist, or nurse practitioner.  You can also be referred to our services if you are being discharged from a hospital or nursing home.

We see patients in Clinton and surrounding counties.

Our nurses can do wound care, obtain lab specimens (we have a partnership with LabCorp), give shots, administer IV therapy, change urinary catheters, drain chest tubes using Pleurex catheters, and perform many other procedures in the comfort of your home.

Everyone we serve receives help managing their medications, teaching about your health (including things like how to manage symptoms of your diseases, about your medications, ways to move around safer in your home, and ways to help reduce your pain), and ongoing monitoring by a team of nurses.  We help to coordinate care with your doctors and pharmacy to ensure you get the best medical care.
